What's New?
In an effort to better accommodate our customers and simplify the registration process, we will no longer have a priority registration period when signing up for recreation classes. All registration will be accepted online, in person, and via mail and fax on a first come, first served basis. Please note that mail or fax registration may not be processed as quickly as online or in person registration.
We will begin accepting online, in person, fax, and mail registration on
Tuesday, March 20, 2012 at 8:00 a.m.
We will begin accepting phone registration on Monday, April 2, 2012
(if you have a current liability waiver form on file).
The City of Davis Community Services Department, located in City Hall at 600 A Street., Suite C is open Monday through Friday from 8:00 a.m. – 5:00 p.m., including the noon hour. You can call us during business houre at (530) 757-5626. Our fax number is (530) 750-2742.

Recreation Program Forms & Handouts
Class Handouts & Parent Handbooks
Many of our classes have handouts containing activity calendars and event schedules. Parent Handbooks are available for most camps, covering activities, appropriate clothing and equipment, pick up and drop off locations, and much more..
Class Registration Form and Liability Waiver 
Use this form to register for recreation programs and classes. For more information on how to register for classes, and our registration, transfer, refund, and late pick-up policies, please visit the How To Register For A Class page.
Disabled Individuals Verification Form - Warm Water Swimming
Use this form to verify your eligibility for the Warm Water Swimming program.
Fee Subsidy Application Packet
We offer a Fee Subsidy Program that grants a 50% subsidy of class fees to allow low income families to enjoy our recreation classes and programs.
Satisfaction Guaranteed!
If you attend the first day of class and are not fully satisfied, come to the Parks & Community Services office no later than 24 hours after the first class and fill out a Satisfaction Guaranteed Form. A full refund will then be issued. We cannot issue refunds to those who do not attend one full class or program and certain programs are not included.
Special Interest Class Proposal
We are always looking for new, interesting, educational, and exciting classes to offer. If you have a particular interest or expertise and wish to teach a class, download our Special Interest Class Proposal packet, fill it out, and send it in!

Aquatics User Group Reservation Form
This form is used by aquatics groups to request reservation of City swimming pools for programs and activities.
