Welcome to the My Davis service request system! 

My Davis is a citizen request management system that provides a 24/7 reporting system for items, such as a pothole that needs filling, graffiti that needs to be removed, street light outages or water leaks.

You can submit comments, concerns and service requests 24 hours a day, seven days a week. Your request will automatically be sent to the correct staff member and department, and you will receive a tracking number and a response shortly after submission, with an estimated resolution timeline.

Requests can be submitted either by using the form below or by downloading the My Davis app from the Apple App Store or the Google Play store.

The City often receives comments from residents who are just looking for more information on a particular subject. Before submitting a request, you may want to search the website by topic to see if there is a page devoted to your area of interest.

Please note this is not an emergency reporting system. If you need to report an in-progress or life-threatening event, please call 9-1-1.
